To request for copies at this level, the searcher would first have to download the relevant form and fill up the same. Note that the request form for a certified copy is different from the request form for an informational copy, so the searcher must be careful when downloading. Note that requests for certified copies would require the searcher to attach a notarized sworn statement to the form, the format of which would be included in the form to be downloaded. In all other respects, the procedure is the same as at the local level, though the required fee is fifteen dollars per copy of the document requested.
